The Arizona Department of Health Services is the only agency that can issue licenses for adult day care centers in the State of Arizona. In order to obtain a license, applicants must pass all parts of the licensing examination. This includes the written, oral and health inspection components of the exam. The written portion of this exam consists of 50 multiple choice questions, as well as two performance based questions that test your ability to assess patients in various situations. In addition to passing this section of the exam, you will also need to successfully complete an oral interview with an ADCHP examiner.
